The lever is the blade you pull. The perch is the clamp and pivot it sits in on the bar. A replacement lever fits your stock perch; a perch kit replaces the lot, usually with a folding lever included.
Most replacement folding levers are made to fit the stock perch for the bike and year on the listing. If the bike has an aftermarket perch, check the brand of the perch first.
